SCIENCE-BASED TREATMENT REQUIREMENTS to
ELIMINATE the WIDESPREAD MISMANAGEMENT of PERIODONTITIS

In 2018 the American Academy of Periodontology and European Federation of Periodontology updated the classification system for periodontal disease that includes staging and grading levels. Recently published papers by Harrel et al. have melded those guidelines with clinical decision criteria and appropriate treatments and maintenance needs—based on the scientific literature and decades of documented successful treatment of periodontitis in clinical practice. Dental standards no longer allow for incomplete scaling and root planing and merely observing progressive disease.

The Cobb and Sottosanti paper, “A re-evaluation of scaling and root planing” published in the Journal of Periodontology (2021), concludes that scaling and root planing must be performed to the degree that pocket depth are reduced, roots are smooth, and the clinical signs of inflammation are absent. If not, further treatment recommendations are necessary. Periodontitis is all too often maltreated, including incomplete treatment, lack of timely and ongoing reevaluations, and lack of advanced treatments when indicated. Any of these circumstances may cause harm to the patient.  The goal of the panel discussion is to confront the practice philosophy that leads to inadequate periodontal therapy and to present an approach that will protect patients and clinicians alike.

Dr. Sottosanti received his dental degree from Georgetown University and his graduate training in periodontics from the University of Southern California and the West Los Angeles VA Hospital Center. For 27 years he ran the Annual USC Periodontal Symposium. Besides being a part-time faculty member at USC, he spent time teaching at Loma Linda and UCLA. He has performed original research on diseased cementum and bone regeneration, and has published many scientific articles in peer-reviewed journals. Dr. Sottosanti is a past-president of the California Society of Periodontists, was a six-year member of the Board of Trustees of the American Academy of Periodontology, and has had a longtime private practice limited to periodontics and implantology in La Jolla, California

Mr. Levy is the founder and owner of the Levy Law Firm which handles dental malpractice cases throughout the United States. He attended the USC School of Dentistry prior to obtaining his law degree from Pepperdine Law School. Previously he received a bachelor of science degree in Biological Sciences from the University of California, Irvine. Over the past 22 years, he has recovered over $100 million for families and victims of dental malpractice including many wrongful death cases. In addition to handling  his own case load, he serves as a consultant for many other attorneys and law firms around the United States on dental malpractice matters.

Dr. Sheldon is a periodontist practicing in Melbourne, Florida. He is a graduate of the Tufts University School of Dental Medicine and received his specialty training from the University of Connecticut School of Dental Medicine. He has developed the concept of “Director of Dentistry” for his fee for service three-doctor practice. As the senior doctor, his role is to examine patients and develop their treatment plan. The treating doctors, a periodontist and a restorative dentist, in consultation with Dr Sheldon, carry out the treatment plans. Consultation with outside specialists, when necessary, is part of the process. Dr. Sheldon, with his longtime office manager, Danyel Salmela, run the Institute for Dental Specialists, a mentoring and consulting organization.

She is coauthor of Periodontal Instrumentation and has contributed to Carranza’s Clinical Periodontology. She was Editor-in-Chief of Dimensions of Dental Hygiene and is now Co-Director of the Pattison Institute which offers online videos, lectures and hands-on workshops. An internationally recognized speaker on Advanced Periodontal Instrumentation, in 2005, she received the Pfizer ADHA Excellence in Dental Hygiene Award and the USC School of Dentistry Alumnus of the Year Award. In 2006 she received the California Society of Periodontists Award and in 2010 she was inducted into the USC School of Dentistry Hall of Fame.
